In
2004 and 2005, four patients in a French hospital died
of a radiation overdose and nineteen others fell ill.
Their technicians had given them far more exposure to
x-rays than medical practice allows. This didn't
happen by accident but because of a very specific
problem: The hospital incorrectly translated the
operating instructions for the computer software
running the x-ray machines. In 2007, the French
government promised to take action against the
hospital for the mistake.
